We are thrilled to be able to open back up and start seeing your smiling faces again! On behalf of the entire Surprise Orthodontics team, thank you for your understanding during this difficult and often confusing time. I am happy to report that our team, including immediate family members, are in good health. We hope that you and your family are as well.
As we resume normal business hours, you will notice some changes we have made to keep everyone in our office safe. Our commitment to hospital-level infection control has never wavered and during this time when there are still many questions, we have implemented some additional practices to ensure your safety and comfort. Here is what you can expect:
SCREENING – A health screening questionnaire and supplemental informed consent form are required before being seen in our office. Both forms are available here: HEALTH-QUESTIONNAIRE-INFORMED-CONSENT. They will need to be printed out and signed by the patient or responsible party (if patient is a minor) and brought to our office or emailed to
POTENTIAL SICKNESS – If within the last 14 days you have had a fever, any cold or flu-like symptoms, or have had close contact with anyone who has had cold or flu-like symptoms, please let us know and we will reschedule your appointment.
PATIENTS ONLY – Please have additional siblings or guests wait in the car. A parent or guardian may accompany minor children and will be required to fill out the health screening questionnaire and informed consent prior to the appointment.
PHYSICAL DISTANCING – Please maintain the recommended 6 feet physical distance from others while inside the office, whenever possible. We will do our best to manage appointments to allow for social distancing.
HAND SANITIZER – We have hand sanitizer available for use prior to entering the office.
AMENITIES – To reduce the spread of germs, we will not be offering magazines, children’s toys, video games or coffee for the time being.
VIRTUAL APPOINTMENTS – We are offering virtual appointments as an option for certain types of appointments. If you would prefer a virtual appointment via Zoom, please email us at firstname.lastname@example.org.
Much of this is new and different for us all and as the guidelines change, we will continue to make modifications to our procedures. One thing that will not change is our commitment to providing great service to our patients and their families in a safe and welcoming environment.
We look forward to seeing you again!
